MINOR PARTICIPANT - PARENT/LEGAL GUARDIAN CONSENT & AUTHORIZATION

This form must be completed by the parent or legal guardian of a participant who will be under 18 years of age on the date of training.

By completing and electronically signing this form, the undersigned represents that they are the minor participant's parent or legal guardian and have the legal authority to provide the permissions and authorizations contained in this form.

This form does not replace the Minor Participant's Participant Registration & Eligibility Form or any separate waiver, acknowledgment of risk, safety agreement, or other documentation required by Argus Tactical LLC.

MINOR PARTICIPANT INFORMATION

PARENT/LEGAL GUARDIAN AUTHORIZATION & CONSENT

Please review and acknowledge each statement below. By completing and signing this form, I represent that I am the minor participant's parent or legal guardian, or otherwise possess lawful authority to provide the permissions stated below.

1. AUTHORITY TO PROVIDE CONSENT

I represent that I am the parent or legal guardian of the minor participant identified in this form, or otherwise possess lawful authority to provide this consent and authorization. I certify that no court order, custody arrangement, or other legal restriction known to me prohibits me from providing this authorization.

2. AUTHORIZATION TO PARTICIPATE

I give permission for the minor participant identified in this form to participate in the applicable Argus Tactical LLC firearms training course, including classroom instruction, demonstrations, practical exercises, firearm-safety instruction, and other course activities appropriate to the course and the participant.

3. FIREARM AND AMMUNITION HANDLING

Subject to applicable law, instructor authorization, range rules, and Argus Tactical safety requirements, I authorize the minor participant to possess, handle, load, unload, and use firearms and ammunition as reasonably necessary to participate in the applicable training activities.

4. LIVE-FIRE AUTHORIZATION

If live-fire training is included in the course, I authorize the minor participant to participate in live-fire exercises under the direction and supervision required by applicable law, range rules, and Argus Tactical safety procedures.

5. ADULT ACCOMPANIMENT AND SUPERVISION

I understand that applicable law may require the minor participant to be accompanied or supervised by an adult during certain firearms-training activities. I agree that the minor participant will not participate in firearm-handling or live-fire activities unless all applicable accompaniment and supervision requirements are satisfied.

ADDITIONAL SUPERVISION REQUIREMENT — PARTICIPANTS UNDER AGE 12

Virginia law imposes additional supervision requirements when a child under 12 years of age is authorized to use a firearm. If the minor participant will be under 12 on the date of training, the minor may use a firearm only while under the supervision of an adult who satisfies applicable Virginia law.

For this purpose, Virginia law defines the supervising adult as a parent, guardian, person standing in loco parentis to the child, or a person 21 years of age or older who has permission from the parent, guardian, or person standing in loco parentis to supervise the child in the use of a firearm.

PARENT/LEGAL GUARDIAN REPRESENTATIONS

1. MINOR'S LEGAL ELIGIBILITY

To the best of my knowledge, the minor participant is not prohibited by applicable law, court order, custody restriction, or other legal restriction from participating in the firearms-training activities authorized by this form. I understand that Argus Tactical LLC is relying upon this representation and is not independently determining or guaranteeing the minor participant's legal eligibility.

2. ACCURACY OF INFORMATION

I certify that the information I have provided in this form is true and accurate to the best of my knowledge and that I will promptly notify Argus Tactical LLC if any information or representation contained in this form becomes inaccurate before or during the minor participant's training.

3. COMPLIANCE WITH SAFETY AND INSTRUCTOR DIRECTIONS

I understand that the minor participant must comply with all Argus Tactical safety rules, instructor commands, range rules, and applicable laws. I understand that Argus Tactical may restrict, suspend, or terminate the minor participant's participation when reasonably necessary for safety, compliance with applicable law, range requirements, or violation of Argus Tactical policies.

4. CONSENT DOES NOT OVERRIDE LAW

I understand that my consent does not authorize conduct that is otherwise prohibited by law. All firearm possession, handling, transportation, and use by the minor participant remain subject to applicable federal, state, and local law and any applicable range requirements.

FINAL PARENT/LEGAL GUARDIAN DECLARATION & AUTHORIZATION

By signing below, I certify that I have personally reviewed this form and each authorization, representation, certification, and acknowledgment contained within it.

I certify that the information I have provided is true and correct to the best of my knowledge and that I have the legal authority to provide the consent and authorization contained in this form.

I voluntarily authorize the minor participant identified in this form to participate in the applicable Argus Tactical LLC training activities, subject at all times to applicable law, instructor authorization, range rules, safety requirements, and any required adult accompaniment or supervision.

I understand that Argus Tactical LLC relies upon the representations made in this form but does not independently determine, certify, or guarantee the legal eligibility of the minor participant or the undersigned.

I understand that completion of this form does not override or create an exception to any applicable federal, state, or local law.

I understand that Argus Tactical LLC may refuse, restrict, suspend, or terminate the minor participant's participation when reasonably necessary to comply with applicable law, range requirements, Argus Tactical policies, or safety requirements.

I acknowledge that this consent and authorization applies only to the training identified in this submission and does not constitute a general authorization for the minor participant to possess, carry, transport, or use firearms outside the applicable training activities.
